Register with us as a New Patient

How to Register

Patients wishing to register with practice must:

  • Live within the practice area
  • Agree to the Practice Rules.

Patients from overseas will also be asked to bring their passport and, if they are registering as a student, proof of their enrolment at a college/university.

We register patients at the following times:

9:30am to 12:00pm – Monday to Friday 
2:30pm to 6.00pm – Monday to Friday 

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

Patients from overseas will also be asked to bring their passport and, if they are registering as a student, proof of their enrolment at a college/university.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We can only register patients who currently live within our catchment area. Please enter your postcode below to check if your address is within our area.

Step 1: Register Online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

However please note that you will need to upload Photo ID and proof of address documents down below for your registration to be complete. Go to Step 2.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Step 2: Upload ID Documents

Upload scans of registration documents (photo identity and proof of address).

  • Upload photo ID (Government provided identities with your photograph e.g. passport data page, driving licence, etc.)
  • Upload proof of address (Letters from recognised establishments, e.g. council tax letters, utility bills, letters from the hospital, HMRC, tenancy agreement, etc.)

Note: Failing to upload your registration documents could delay or discontinue your registration process. If for any reason you are not able to provide one, our reception team will be happy to talk you through the alternatives, depending on your circumstance.

Please complete all relevant sections on the registration form(s) and hand in at the Practice Reception with your registration documents.

The National Care Record Service (NCRS)

Important information regarding your NHS medical records – Summary Care Record

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
